ONE DAY THESE GIRLS WILL BE FILLING YOUR SHOES ON YOUR SCHOOL FIELDS. HELP US MAKE SURE THEY'RE READY.

We're looking for high school and select-level softball players to help coach at our Fall Softball Prep Clinic in Bonney Lake this August. This is your chance to give back to the game that gave you everything, and help the next group of players coming up behind you.

You'll work alongside experienced coaches, helping young athletes ages 7-13 learn the fundamentals of the game. These girls look up to players like you. Your presence, your encouragement, and your knowledge make a difference they'll carry with them.

Please read the following carefully. This must also be reviewed and agreed to by your parent or guardian.

Assumption of Risk

I understand that softball involves inherent risks including but not limited to: being struck by a ball or bat, contact with other participants, falls, muscle strains, sprains, fractures, and other injuries. I voluntarily assume these risks when volunteering.

Release of Liability

In consideration of being permitted to volunteer, I release CoachPilot LLC, its coaches, officers, employees, and volunteers, along with the Sumner-Bonney Lake School District, from any and all claims, demands, or causes of action arising from my participation, except in the case of gross negligence or willful misconduct.

Photo & Media Release

I grant CoachPilot LLC permission to use photographs and video of me taken during clinics for promotional, educational, and marketing purposes (website, social media, print materials). My name may be used with the media unless I opt out below.

Code of Conduct

I agree to conduct myself as a role model for the young athletes. This means: no profanity, no bullying, no physical roughhousing, no personal contact with participants outside of scheduled clinic activities, no photography of participants on my personal devices, and full respect for coaches, parents, and other volunteers.

Emergency Medical Treatment

In the event of a medical emergency, I authorize CoachPilot LLC staff to seek emergency medical treatment on my behalf and to contact my parent or guardian at the number provided.

Youth Protection

I understand that I will be around minors. I agree to always operate under the two-adult rule (never alone with a single participant), to report any concerns to a coach immediately, and to maintain appropriate boundaries at all times.

Concussion & Cardiac Awareness

I acknowledge I have completed the required concussion and sudden cardiac arrest education in compliance with Washington RCW 28A.600.190 and RCW 28A.600.195, and I understand my responsibility to report any suspected head injury or medical emergency to the head coach immediately.

Everyone under 18 needs a parent's approval to volunteer. If your parent is here, they can sign this form with you. If not, we'll email them a link to approve it.
