LIABILITY RELEASE & ASSUMPTION OF RISK

In consideration of my child's participation in the Fall Softball Prep Clinic ("Clinic") operated by CoachPilot LLC, I acknowledge and agree to the following:
Assumption of Risk. I understand that softball and related athletic activities involve inherent risks including but not limited to: contact with bats, balls, and other equipment; collisions with other participants; falls on playing surfaces; muscle strains, sprains, fractures, and other injuries; exposure to weather conditions including heat, sun, rain, and lightning; and other hazards inherent to outdoor athletic activity. I voluntarily assume all such risks on behalf of my child.
RELEASE AND HOLD HARMLESS. I, on behalf of myself, my child, and our heirs, executors, and assigns, hereby RELEASE, WAIVE, DISCHARGE, AND HOLD HARMLESS CoachPilot LLC, its owners, coaches, volunteers, assistants, and affiliated personnel from any and all liability, claims, demands, or causes of action arising out of or related to any injury, illness, or damage sustained by my child during or as a result of participation in the Clinic, except in cases of gross negligence or willful misconduct.
INDEMNIFICATION. I agree to INDEMNIFY AND HOLD HARMLESS the released parties from any claims, damages, or expenses (including attorney fees) brought by or on behalf of my child or any third party related to my child's participation.
I confirm that my child is physically fit to participate and has no medical conditions that would prevent safe participation, unless disclosed in the medical section below.
CONCUSSION INFORMATION
Required by Washington State Law (RCW 28A.600.190)
The Zackery Lystedt Law requires that coaches, parents, and athletes receive information about the signs and symptoms of concussion and understand the return-to-play protocol.
What is a concussion? A concussion is a brain injury caused by a bump, blow, or jolt to the head, or a hit to the body that causes the brain to move rapidly inside the skull. Even a "ding" or what seems like a mild bump can be serious.
Signs and symptoms may include:
- Headache or pressure in the head
- Nausea, vomiting, or dizziness
- Balance problems or feeling unsteady
- Double or blurry vision
- Sensitivity to light or noise
- Feeling sluggish, foggy, or groggy
- Difficulty concentrating or remembering
- Confusion or feeling "not right"
- Loss of consciousness (even briefly)
Clinic policy: Any athlete who exhibits signs, symptoms, or behaviors consistent with a concussion will be immediately removed from participation and will not be allowed to return until evaluated and cleared in writing by a licensed healthcare provider trained in concussion evaluation and management.
By signing below, both the parent/guardian and athlete acknowledge that they have read and understand this concussion information.
SUDDEN CARDIAC ARREST AWARENESS
Required by Washington State Law (RCW 28A.600.195)
Washington state law requires that coaches, parents, and athletes receive information about sudden cardiac arrest (SCA) before participating in youth athletic activities.
What is sudden cardiac arrest? SCA is a life-threatening condition where the heart suddenly and unexpectedly stops beating. When this happens, blood stops flowing to the brain and vital organs. If not treated within minutes, SCA is fatal.
Warning signs and symptoms that may precede sudden cardiac arrest include: fainting or seizure during or after exercise, unexplained shortness of breath, chest pain or discomfort during exercise, dizziness or lightheadedness, racing or fluttering heartbeat, extreme fatigue disproportionate to activity, and any family history of unexplained sudden death before age 50.
Clinic policy: Any athlete who exhibits warning signs of SCA will be immediately removed from participation and emergency services will be called. The athlete will not be allowed to return until evaluated and cleared in writing by a licensed healthcare provider. If an SCA occurs, our staff is trained to call 911 and administer CPR and AED as needed.
By signing below, both the parent/guardian and athlete acknowledge that they have read and understand this sudden cardiac arrest information, as required under Washington's HB 1824 / SB 5083 Youth Sports Head Injury and Sudden Cardiac Arrest compliance policies.

- Listen to coaches and follow instructions
- Treat teammates, coaches, and opponents with respect
- Give full effort and maintain a positive attitude
- Use appropriate language at all times
- Take care of equipment and facilities
Parent/guardian expectations:
- Drop off and pick up players on time
- Direct any concerns or questions to clinic staff privately
CoachPilot LLC reserves the right to remove any participant whose behavior is disruptive, unsafe, or inconsistent with these expectations.
